Running Shoes Can Hurt Hips, Knees

Running is indisputably good for many essential body parts–hearts, lungs, and in many cases, attitude, but it may be bad for a few places where one bone connects to another, particularly if the runner is wearing running shoes, as almost all runners do. So says research published in the December 2009 issue of PM&R: The journal of injury, function and rehabilitation.

ScienceDaily reports on the study, which suggests that running shoes exerted more stress on hips, knees and ankles than did running barefoot or walking in high-heeled shoes. The researchers observed 68 young healthy runners, who wore currently available running shoes and at least 15 miles a week. They used a treadmill and a motion analysis system, and observed each subject running barefoot and with shoes. ScienceDaily reports that the scientists found increased joint torques at the hip, knee
and ankle with running shoes compared with running barefoot.
Disproportionately large increases were observed in the hip internal
rotation torque and in the knee flexion and knee varus torques. An
average 54 percent increase in the hip internal rotation torque, a 36 percent
increase in knee flexion torque, and a 38 percent increase in knee varus
torque were were measured when running in running shoes compared with barefoot. The researchers concluded that while their findings confirm that modern-day running shoes provide good support and protection of the
foot, they also increase stress on each of the three lower extremity joints. These increases are likely caused in large
part by an elevated heel and increased material under the medial arch,
both characteristic of today’s running shoes. What’s the answer? One solution may be to try running barefoot, or running in these.

  1. Is there anything one can do that isn’t harmful? Let’s see, moving our bodies at a rapid pace where our joints are involved shows some wear. Who would have guessed. Interesting, since I read another university study six months ago indicating the opposite, that even long-term runners showed no more knee/hip/ankle problems than those who don’t run. I think I will accept the tradeoff.

  3. After reading Born to Run by Chris McCougall, I started thinking about my shoes. I don’t run much, but I’m running more after developing recurring elbow and forearm tendonitis on a rowing machine. I’ve been a little afraid to raise my mileage or intensity, because I don’t want to hurt my legs, too. I just ordered a pair of Nike Free 5.0. I want something on my feet. I’m hoping that my feet and legs will become stronger after I get used to the shoes. My daughter, who ran CC and track in traditional shoes in high school, got Vibram Five Fingers and after some initial discomfort she says she won’t go back. They’re a little radical for me, but I’m willing to experiment.

  4. I have been training barefoot for 11 years through my program willPower & grace. I’ve also run marathons and 1/2 marathons in the Frees (5.0, 4.0 and 3.5). Training barefoot strengthens your entire kenetic chain, from the ground up. It’s functional training! Jumping and running produce impact – which can be good – essentially the overload principle. The key is: use your core and learn to decelerate. Pay attention to how you land on your feet. TRUST your feet!
